The Form 4013: Resident Tribal Member Annual Sales Tax Credit is available upon request through the LTBB Dept of Commerce.
Please contact the LTBB Dept of Commerce at 231-242-1584 or DOC@ltbbodawa-nsn.gov if you would like a copy mailed or emailed to you.
This is a form for LTBB Tribal Citizens who lived in the LTBB Tax Agreement Area during the 2022 tax year, and is used to obtain your annual sales tax credit.
