- Eligibility Confirmation– This checklist is used to determine if a client is eligible for WMDCP. It is filled out by the Coordinator and the Prosecutor.
- Confidentiality Statement– This contract is filled out and signed by visitors of our program events or hearings. It is used to ensure the confidentiality of program participants.
- Client Contract– This contract is filled out and signed by a new drug court client and witnessed by the defense attorney. Terms of the contract are explained thoroughly by the new client’s defense attorney.
- Chemical/Alcohol Free Home Agreement– This contract is filled out and signed by WMDCP participants and any adults who live under the client’s household. This agreement is reinforced by random home visits in which court officials check the home for signs of violations.
- Order for Discharge– This form is used when a client is either successfully or unsuccessfully discharged from the WMDCP program. It is filled out by the Coordinator and signed by the Judge.
- Probation Violation– This form is used to notify the court when a client violates the terms laid out in the Client Contract. It is filled out by the probation officer, stamped by the Judge and is served by the Probation Officer.
- Entrance Questionnaire– This form is used to gather initial client data and form a baseline for client success. It is with this form and the accompanying interview that a client history is established. This form is filled out by a new client and the Program Coordinator.
- Exit Questionnaire– This form is used to gather information regarding the client’s participation in the WMDCP. The data gathered in this questionnaire is compared with the data gathered in the Entrance Questionnaire to measure client growth and accomplishments. This form is filled out by the client and the Program Coordinator.
- Notice of Unscheduled Hearing– This notice is used when a non-regular hearing needs to take place. For example, if it is discovered during the week that a drug court client has violated the Client Contract, immediate action must be taken and an emergency hearing must be called. It is prepared by the Program Coordinator and served by the Probation Officer or Law Enforcement.
- Order Following WMDCP Hearing– This order is prepared following each drug court hearing and it spells out exactly what is expected of the drug court participant for that period until their next hearing. The order indicates what week of the WMDCP curriculum that a client is to complete and when they will be expected to attend the next drug court hearing. It is prepared by the Program Coordinator and served by the Probation Officer or Law Enforcement.
- Progress Report– This report is filled out before each drug court hearing. It is meant to be a snapshot of the drug court client’s progress for the period. The report outlines the components of the WMDCP Success Formula. The team members are all given copies of the report before the weekly team meeting. The report is prepared by the Program Coordinator.
- Release of Confidential Information– This agreement gives the Court authority to share information with appropriate agencies in order to coordinate services for the client. It is filled out and signed by the defendant and the Program Coordinator.
- Request for Travel– This form is filled out by the drug court participant when they are requesting to travel outside of the agreed upon three county area of Charlevoix, Emmet, and Cheboygan counties. A form must be filled out and signed by an appropriate court employee before a client is allowed to outside this area.
- Standard Terms of Probation– This form is signed by the drug court participant upon entry to the WMDCP and witnessed by the Probation Officer. It spells out the standard terms of probation.
