THIS IS A REQUIREMENT OF THE PROGRAM

The regulations state if you reside in the 27-count/es, you must utilize the LTBB Dental Clinic

Establish an appointment with the LTBB Dental Clinic

THE DENTIST Will DETERMINE WHAT TREATMENT NEEDS TO BE DONE

The Dental Front Desk Assistant will supply an Elders Dental Program Application, if needed

Complete Elders Dental Program Application in its entirety

INCOMPLETE APPLICATIONS Will BE DENIED

Ensure you read and fill out the application completely Failure to do so wifl result In a denial and a new application must be resubmitted

The application will be reviewed by the Health Services Navigator

YOU Will RECEIVE A LETTER IF IT HAS BEEN APPROVED OR DENIED

If approved, you may begin scheduling your appointments with the LTBB Dental Clinic

Do NOT have any services performed before applying

YOUR APPLICATION WILL BE DENIED

The regulations state if you MUST apply prior to any services completed

Discuss with your Dentist about accepting payment from LTBB

NOT ALL PROVIDERS ARE WILLING TO ACCEPT PAYMENT FROM LTBB
