LTBB COVID-19 Vaccination Sign-Up and Information

COVID-19 Vaccines will be administered by appointment only at the LTBB Vaccine Clinic at Courtyard by Marriott in Petoskey, MI. You can get on our vaccine list by clicking the appropriate link below and completing the sign-up form. By submitting this form, your name will be added to the list of people who want the vaccine, and the LTBB Health Department staff will contact you to schedule your appointment once vaccines become available. Please only submit one request per person. Submitting multiple requests will only slow our vaccination efforts. Miigwetch (Thank You)!

LTBB is Holding a COVID-19 Vaccine Clinic in Lansing!

 

Date: Saturday, April 17, 2021

Appointment times: 9:00 am – 4:30 pm

Location: New Covenant Christian Church and School

Who can get a vaccine?  Open to everyone who is age 18 years or above.

We will be using the Moderna vaccine at the  LTBB Lansing Vaccine Clinic this Saturday and we will be returning to Lansing for the booster clinic on May 15th.

 

Here is how to get an appointment for the vaccine clinic:

Step 1:

Step 2:

Within a few days, a health screening form will be texted to you on your phone.  Click the link sent to you and answer the questions and tap SUBMIT.

Step 3:

Once your health form is reviewed, a scheduler will contact you to schedule you an appointment.  Please be patient!  You will hear from us.

LTBB Vaccine Sign Up-Tribal Community

Our main goal continues to be vaccinating our Tribal Citizens and Tribal Community. People in this category will continue to receive priority in vaccine appointment scheduling.

This sign up should be used by all Tribal Citizens of federally recognized tribes, their descendants, and those living in the households of these individuals.

 

LTBB Vaccine Sign Up- Service Workers

“Since the beginning of the pandemic, employees working in the service industry have been on the front lines, continuing to provide important services to the public, putting themselves at risk. We are pleased to be able to offer vaccines to this sector of employees who are so critical to the economy of our area” –LTBB Tribal Chairwoman, Regina Gasco-Bentley

 

This sign up should be used by service workers that work in Emmet, Charlevoix, and Cheboygan counties. People in this category will continue to be prioritized over the general public in vaccine appointment scheduling.

 

LTBB Vaccine Sign Up- General Public

As of March 23rd, the LTBB Health Department has expanded eligibility to include anyone living in Emmet, Charlevoix, and Cheboygan counties, aged 16 and up.

Post-vaccination Information

If this is an emergency, please dial 911 or seek out emergency care at the nearest Emergency Room.  All Vaccine associated reactions, adverse events should be reported to your Primary Care Provider, or seek urgent/emergency care as needed.  Report all vaccine related reactions by signing up for VSafe https://www.cdc.gov/coronavirus/2019-ncov/vaccines/safety/vsafe.html recommended per CDC in order to monitor Emergency Use Authorized vaccines and associated reactions/side effects.
