Due the closure of the Administration Building Enrollment asks that all Address Change and other requests be sent via email to firstname.lastname@example.org. The reception of US Mail and faxes will be extremely delayed during the closure (until November 8).

We appreciate your patience and understanding. We are working hard to make sure all address changes are processed in a timely manner to meet the Per Capita deadline and to support the Payment IV Program with Human Services.
